Process for NDIS therapist collaboration with Springvale Park SDS staff:

1. Download the two forms below and return them to ndis@springvaleparksds.vic.edu.au
   • Ensure that both the Parent and Carer Consent Form and the Request Forms are filled out completely and signed.
   • Therapist are required to provide evidence of their Working With Children Check and Certificates of Currency for public liability insurance and professional indemnity insurance.
2. Our NDIS Navigator will review your requests and notify you if your request has been approved.

Important information about NDIS at Springvale Park SDS

>Due to facility constraints and to avoid disruption to classroom learning programs, Springvale Park SDS does not permit on-site therapy visits by NDIS therapists.

>We are happy to arrange a meeting, email or telephone communication between NDIS therapists and our staff in most cases.

>Due to large number of requests we are receiving for classroom observations, and the disruption associated with classroom visits – we are not currently approving classroom observations for NDIS therapists.
